Well the second is obviously not real. That cast is way too huge on him and I think that you would have posted if something that freaky had happened to him.
Going with a play cast. Way too big for such a little guy. Plus, if a doctor tried to put a cast like that on my kiddo I think I'd get a second opinion. :)
